Step-by-Step: Getting Your First U.S. Passport Under Chok Hasaba (Grandparent Clause)

If your child is eligible for U.S. citizenship through the "Chok Hasaba" or Grandparent Clause, here’s a detailed guide on getting their first U.S. passport:


*This process starts once the child gets citizenship in the US with the grandparent clause.


1. Both Parents Must Be Present  

   Both parents are required to be present at the appointment. If one parent cannot attend, a notarized DS-3053 form is required. Form DS-3053 https://eforms.state.gov/Forms/ds3053.pdf


2. Documents to Bring  

   - DS-11 Passport Application: Fill it out in black ink without signing it until instructed at the embassy. DS-11 Form https://pptform.state.gov/passportwizardmain.aspx.


   - Birth Certificate: Original and three copies. If it’s in Hebrew, translate it.

   - Marriage Certificate: Original, one copy, and translation.

   - Passport Photos: Three square photos.

   - Proof of Residency: Immunization forms, dental records, or school transcripts.

   - Appointment Confirmation: Print this to prove your slot.

   

3. Paying Ahead  

   You can now pay online ahead of the appointment! Be sure to print your payment confirmation and the appointment slot.
