How to Apply for a Child’s First U.S. Passport Abroad
Applying for your child’s first U.S. passport through the U.S. Embassy in Israel? Here’s a clear step-by-step guide to get it done right.
Step 1: Fill Out the DS-11 Form
Start with the official DS-11 passport application form.Complete it online, print it out, and do not sign it until your appointment at the embassy.
Step 2: Take a Passport Photo
Take a recent passport photo of your child that meets U.S. guidelines.
Step 3: Book Your Appointment on USTravelDocs
Go to USTravelDocs Israel and create an account.
Choose CRBA (includes birth certificate, passport, and SSN).
Select pickup (free) or courier delivery. Courier is paid separately—see below.
Print your appointment confirmation, which includes your UID number.

Step 5: Prepare Required Documents

Bring originals and three black-and-white copies of each item:
Child’s birth certificate
Passport photo
Parents’ marriage certificate (if applicable)
Parent(s)’ U.S. passport / proof of citizenship
Non-U.S. parent’s passport or ID
Proof of U.S. citizen parent’s physical presence in the U.S.If any document is in Hebrew, self-translate to English—no notary needed.
Step 6: Embassy Appointment
Arrive on time.
Bring all documents + 3 copies of each.
Present your appointment confirmation.
Sign the DS-11 only when the consular officer instructs you to.
