Checklist: SSN First-Time Applicant (Over Age 12)

Before Your Appointment:☐ Schedule an in-person interview – Appointments are only available at the U.S. Embassy Jerusalem FBU office.

☐ Prepare all original documents, photocopies, and handwritten translations – No photocopies will be made at the interview, and incomplete applications will not be accepted.

☐ Print this checklist and place it in front of your application package.

Required Documents

✅ 1. Completed Application for a Social Security Card (Form SS-5-FS)

All questions must be answered and legible.

✅ 2. Completed Interview Sheet

Do not sign it before the appointment.

✅ 3. Proof of Birth (One of the following):☐ Birth certificate☐ U.S. Consular Report of Birth Abroad☐ If born in the U.S.: Form SSA-L706 (Page 1/Part 1 – attached) must be completed, signed, and submitted. This allows SSA to verify the birth record.☐ Proof of name change (if applicable): Marriage certificate, Name Change Certificate, Court Order, or Naturalization Certificate.

✅ 4. Proof of Prolonged Stay Outside the U.S.☐ Original documents showing a prolonged stay outside the U.S. (e.g., school records, employment records, medical records).

✅ 5. Proof of U.S. Citizenship or Lawful Work-Authorized Immigration Status (One of the following):☐ U.S. birth certificate☐ U.S. Consular Report of Birth Abroad☐ U.S. Naturalization Certificate☐ U.S. Citizenship Certificate☐ U.S. Passport☐ Valid Lawful Permanent Residence Card

✅ 6. Proof of Identity (One of the following):☐ Original valid and signed U.S. Passport☐ Valid U.S. Permanent Residence Card☐ Current and signed Israeli Passport☐ Israeli National ID☐ Valid Palestinian Authority Passport☐ Palestinian Authority ID card

Important Information:

Processing time: 8 to 10 months to receive the Social Security card.
